ABSTRACT

This study investigated the effects of a foot core intervention on the coordination of foot joints in recreational runners. This was a secondary analysis from a randomized controlled trial conducted with 87 recreational runners allocated to the control group (CG), which followed a placebo lower limb stretching protocol, or the intervention group (IG), which underwent an 8-week (3 times/week) foot core training. The participants ran on a force-instrumented treadmill at a self-selected speed (9.5-10.5 km/h) while the foot segment motion was captured. The vector coding technique was used to assess inter-joint coordination for four selected coupled segment and joint angles. The coordination patterns of the calcaneus and midfoot (CalMid) and midfoot and metatarsus (MidMet) joint pairs were affected. In the frontal plane, IG showed an in-phase with proximal dominancy coordination at heel strike, with a decrease in its frequency after the training (P=0.018), suggesting a longer foot supination. Additionally, IG showed an anti-phase with distal dominancy pattern at early stance compared to CG due to a smaller but earlier inversion of the CalMid-MidMet pair (P=0.020). The intervention also had an effect on the transverse plane of the CalMid-MidMet pair, with IG showing a significantly greater frequency of anti-phase coordination with proximal dominancy during propulsion than CG (P=0.013), probably due to a reduction in the CalMid abduction. Overall, the results suggested that the foot core intervention reduces the occurrence of running-related injuries by increasing the resistance to calcaneus pronation and building a more rigid and efficient lever during push-off.

ABSTRACT

Objectives: To examine drug overdose records in Brazil from 2000 to 2020, analyzing trends over time in overdoses and overall sociodemographic characteristics of the deceased. Methods: Using data from the Brazilian Mortality Information System (Sistema de Informações sobre Mortalidade), we identified records from 2000-2020 in which the underlying cause-of-death was one of the following codes: X40-X45 (accidental poisoning), X60-X65 (intentional poisoning), or Y10-Y15 (undetermined intentionality poisoning). The Brazilian dataset included 21,410 deaths. We used joinpoint regression analysis to assess changes in trends over time. Results: People who died of drug overdoses in Brazil between 2000 and 2020 had a mean age of 38.91 years; 38.45% were women, and 44.01% were identified as White. Of the overdose deaths, 44.70% were classified as intentional and 32.12% were classified as unintentional. Among the identified drugs, stimulants were the most common class. However, most records did not report which drug was responsible for death. Conclusion: Sociodemographic trends in overdose deaths in Brazil must guide country-specific policies. Nevertheless, data collection protocols must be improved, particularly regarding the drug used in overdoses.

El Hospital Garrahan ha sido pionero en el diagnóstico molecular de patologías pediátricas en Argentina. Los avances tecnológicos de las últimas décadas en el área de la biología molecular, sentaron las bases para la optimización y ampliación del diagnóstico molecular a partir de la secuenciación masiva en paralelo de múltiples genes. El presente trabajo describe el proceso de implementación de los estudios de secuenciación de nueva generación y el desarrollo de la Unidad de Genómica en un hospital público pediátrico de alta complejidad, así como su impacto en las capacidades diagnósticas de enfermedades poco frecuentes de origen genético. La creación del Grupo Interdisciplinario de Estudios Genómicos constituyó la vía institucional para la toma de decisiones que implican la implementación de nuevos estudios genómicos y el establecimiento de prioridades diagnósticas, extendiendo la disponibilidad del diagnóstico molecular a más disciplinas. La Unidad de Genómica trabaja en diseñar las estrategias que permitan la mayor optimización de los recursos con los que cuenta el hospital, teniendo en cuenta el equipamiento disponible, las prioridades establecidas y la frecuencia de las distintas patologías. Se demuestra el salto significativo operado en nuestras capacidades diagnósticas, tanto en la variedad de enfermedades como en el número de genes analizados, habiendo estudiado a la fecha alrededor de 2.000 pacientes, muchos de los cuales ven de este modo finalizada su odisea diagnóstica. Los estudios de NGS se han convertido en una herramienta de la práctica diaria para la atención de un número importante de pacientes de nuestro hospital. Continuaremos trabajando para ampliar su aplicación a la mayor cantidad de patologías, a través de los mecanismos institucionales ya existentes (AU)


The Garrahan Hospital has been a pioneer in the molecular diagnosis of pediatric diseases in Argentina. The technological advances of the last decades in the area of molecular biology have laid the foundations for the optimization and expansion of molecular diagnostics through massive parallel sequencing of multiple genes. This study describes the process of implementation of next-generation sequencing studies and the development of the Genomics Unit in a public pediatric tertiary hospital, and its impact on the capacity to diagnose rare diseases of genetic origin. The creation of the Interdisciplinary Group of Genomic Studies constituted the institutional pathway for decision-making involving the implementation of new genomic studies and the establishment of diagnostic priorities, extending the availability of molecular diagnostics to additional disciplines. The Genomics Unit is working to design strategies that allow for optimization of the resources available to the hospital, taking into account the equipment available, the priorities established, and the frequency of the different diseases. It demonstrates the significant leap in our diagnostic capabilities, both in the variety of diseases and in the number of genes analyzed. To date, around 2,000 patients have been studies, many of whom have thus completed their diagnostic odyssey. NGS studies have become a tool in daily practice for the care of a significant number of patients in our hospital. We will continue working to expand its application to as many diseases as possible, through the existing institutional mechanisms (AU)

ABSTRACT

Aims: This study evaluated the physical, chemical and rheological properties of exopolysaccharides (EPSs) produced by Lactic Acid Bacteria (LAB) isolated from palm wine. Materials and Methods: EPSs from palm wine LAB strains were produced on 6% sucrose broth, purified and freeze-dried prior to analyses. Molecular weights (MW), rheological and structural composition (functional groups) of the EPSs were determined using standard methods and Fourier transform infrared spectroscopy (FTIR). Results: The average MW of the EPSs ranged from 2.02×106 to 6.53×106 Da while the flow index (n) values ranged from 0.03-3.13 at 0.2%, 0.06-1.51 at 0.4%, 0.38 - 1.85 at 0.6%, 0.14 - 2.26 at 0.8% and 0.55 - 6.42 at 1% concentrations at elevated temperatures for EPS solutions from the ten LAB species. The FTIR spectrum revealed prominent peaks of various groups of OH (3420 cm-1) and CH3 bending (2090 cm-1) in all the EPSs corresponding to both hydroxyl and amine groups, and aliphatic C-H bonds, respectively. EPS synthesized by Leuconostoc lactis, Lactococcus lactis subsp lactis and Lactiplantibacillus plantarum showed weak absorption peaks (1148 – 1145 cm-1) indicating the C-O-C and C-O bonds, while absorption peaks of Lactobacillus lactis, Lactobacillus acidophilus and Lactiplantibacillus plantarum (1267 – 1253 cm-1) indicated O- acetyl ester and other non-sugar components. Conclusions: The FTIR spectra, rheological properties and molecular weight of EPSs synthesized by the ten LAB strains indicated potentials that could be exploited in different industrial applications, and as stabilizers in food industries

Our objective was to study the influence of water magnetization on herbicide efficiency and to verify whether there is a relationship between control scores and the normalized difference vegetation index (NDVI). In the laboratory experiment, water was subjected to magnetization and evaluated with respect to four characteristics. In the field experiment, plots of Brachiaria grass were subjected to treatments in a factorial scheme (6 × 2 + 1). Six herbicidal factors (doses of glyphosate and glyphosate + 2,4-D) and the magnetization or absence of magnetization of the spray solution were evaluated and compared against the control treatment (without spraying). Weed control assessments were carried out six times. Images were obtained using an embedded multispectral camera to determine the NDVI values. Data related to water characteristics were analyzed using the t test. Weed control and NDVI data were subjected to analysis of variance and are presented in regression graphs. Dispersion analysis of NDVI data was performed according to the control scores. The magnetization process decreased the pH of the water and increased the surface tension, but it did not influence the control scores or the NDVI. As the glyphosate dose was increased, the control scores were higher and the NDVI values were lower. Magnetized water did not affect the biological efficiency of the herbicides, and there was a strong correlation between the control scores and the NDVI values.

ABSTRACT

Nuclear proliferation marker MIB-1 (Ki-67) immunohistochemistry (IHC) is used to examine tumor cell proliferation. However, the diagnostic or prognostic value of the Ki-67 nuclear staining intensity and location, defined as nuclear gradient (NG), has not been assessed. This study examined the potential association between Ki-67 NG and cell cycle phases and its effect on the prognosis of pulmonary typical carcinoid (PTC) tumors. We propose a method for classifying the NG of Ki-67 during the cell cycle and compare the results between PTC, pulmonary adenocarcinoma (PAD), and breast ductal carcinoma (BDC). A literature review and objective analysis of IHC-stained paraffin sections were used to determine the Ki-67 labeling index and composed a stratification of the NG into NG1, NG2, and NG3/4 categories. A semi-automated image analysis protocol was established to determine the Ki-67 NG in PTC, PAD, and BDC. High intraobserver consistency and moderate interobserver agreement were achieved in the determination of Ki-67 NG in tumor specimens. NG1 and NG2 were lower in PTC than in PAD and BDC. Cox multivariate analysis of PTC after adjusting for age and number of metastatic lymph nodes showed that Ki-67 NG1 and NG2 significantly predicted clinical outcomes. The semi-automated method for quantification of Ki-67 nuclear immunostaining proposed in this study could become a valuable diagnostic and prognostic tool in PTC.

ABSTRACT

Une douleur aiguë postopératoire peut être source de complications à court voire à long terme si elle est sous-évaluée et sous traitée. Plusieurs facteurs interviennent à l'exacerbation de cette douleur. Notre objectif était de déterminer les facteurs cliniques influençant la sévérité de la douleur post opératoire immédiate après une césarienne. Méthodes : Une étude type cas-témoin portant sur les patientes césarisées était réalisée en salle de soins post-interventionnels du Centre Hospitalier Universitaire Professeur Zafisaona Gabriel Mahajanga allant de janvier 2019 au décembre 2019. L'étude statistique a été réalisée avec le logiciel Epi-Info® 3.5.4 de l'Organisation Mondiale de la Santé Genève et CDC Atlanta. Les mesures d'association ont été mesurées par le calcul d'Odds ratio. Le seuil de signification statistique (p) a été fixé à une valeur < 0,05. Résultats : Nous avons retenu 32 cas pour 64 témoins. La population d'étude était jeune et la majorité appartenait dans la tranche d'âge de 16 à 29 ans. Les facteurs qui interviennent sur l'intensité de la douleur postopératoire étaient surtout des facteurs liés à la patiente tels que le jeune âge (OR= 3,46 [1,17 - 10,18]; p=0,009), la gestité en l'occurrence la primigestité et la paucigestité (OR=2,77 [1,33 - 5,79]; p=0,002), la primiparité et la pauciparité (OR=5,67 [2,16 - 14,94]; p=< 0,001), l'anxiété préopératoire (OR=5,04 [1,99 - 12,74]; p=0,0003) et l'appartenance à la classe 2 selon la classification de l'American Society ofAnesthesiologists (OR=4,2 [1,49 - 11,76]; p=0,004). ABSTRACT

Une analgésie multimodale est incontournable pour la prise en charge de la douleur post opératoire. L'objectif de notre étude était de montrer l'intérêt de l'utilisation de la kétamine, avec son effet analgésique, parmi les différentes molécules disponibles. Méthodes : Il s'agit d'une étude prospective monocentrique en simple aveugle de 31 cas d'hystérectomie programmée sur une période de 6 mois (mai­octobre 2016) divisée en deux groupes soumis au même protocole anesthésique : un groupe recevant de la kétamine en bolus suivi d'une perfusion continue peropératoire et sur les premières 24 heures et un groupe contrôle sans kétamine. En période post opératoire les deux groupes bénéficient d'une analgésie multimodale. L'analyse statistique a été faite par le test de Student. Résultats : Les scores de douleur diffèrent peu en moyenne sur les 24 heures de surveillance, tandis qu'une épargne morphinique importante est notée dans le groupe kétamine de l'ordre de 50% avec une valeur de p<0,001 fortement significative. A côté nous n'avons constaté aucun effet indésirable notable. Conclusion : L'adjonction de kétamine même de brève durée dans la prise en charge de la douleur post opératoire a permis de baisser considérablement la consommation morphinique

ABSTRACT

Polycyclic aromatic hydrocarbons, PAHs are carcinogenic and genotoxic in nature and have been of worldwide concern. This study aimed at determining the PAH contamination levels in types of wheat (Tritcum specie), bambara groundnut (Vigna subterranea) and pigeon peas (Cajanus cajanifolia) commonly consumed in the eastern part of Nigeria and assess the health risk associated with their consumption. The grain samples were analyzed of sixteen priority PAHs using gas chromatography coupled with flame ionization detector, GC-FID after extraction by sonication. Estimation of daily intakes were carried out using adult male and female consumers while margin of exposure was used to assess the health risk applying bench mark dose levels for the indicators-BaP, PAH2, PAH4 and PAH8. The sixteen PAHs were detected in all the analyzed grains. The ∑16 PAHs concentrations (× 10¯²µg/kg) detected ranged from 25.004±20.553 in white pigeon peas to 36.493±20.305 in red pigeon peas. The eight probable carcinogenic PAHs (∑PAH8) detected ranged from 10.913±4.295 to 17.444±7.023 also in white and red pigeon peas respectively. From the estimation of daily intake calculated, the total dietary exposure of male (41.42 µg/kg bw/day) was less than that of female (48.24 µg/kg bw/day) implying that adult female are more exposed. The MOE for adult male individual ranged from 49,893 in pure white bambara groundnut to 392,943 in pigeon peas. While for adult female individual, the values of MOE ranged from 48,110 in bambara groundnut to 336,770 in pigeon peas. The values of margin of MOE obtained for all the indicators were much higher than 10000 which according to EFSA indicate low concern for human health and considered low priority for risk management actions. The PAHs values detected were all below 1.0 µg/kg which is the permissible limit established by EFSA. Based on these facts, these grains are safe for consumption.

ABSTRACT

Background and Aims: Manotes expansa Sol. ex Planch. and Aframomum alboviolaceum (Ridl.) Schum. are two plants belonging respectively to the family Connaraceae R.Br. and Zingiberaceae Martino widely used in traditional medicine for the treatment of eye diseases, fever, headaches, gastritis as well as asthma. The aim of the present study is the valorization of these two plants collected in the Democratic Republic of Congo by a quantitative and qualitative analysis of secondary metabolites and mineral elements in their leaves. Materials and Methods: The determination of secondary metabolites in the leaves of Manotes expansa and Aframomum alboviolaceum was carried out by UV-Visible spectrophotometry and X-ray fluorescence spectrophotometry for the identification and quantitative analysis of mineral elements. Results: The results showed that the leaves of these two plants are rich in phenolic compounds, i.e. 442.2 mgEqAG/g for the leaves of A. alboviolaceum, 370.64 mgEqAG/g for the red leaves and 282.64 mgEqAG/g green leaves of M. expansa. Although being part of the same plant, the red and green leaves of M. expansa presented a totally different phytochemical profile. The contents of condensed tannins, anthocyanins and flavonoids are respectively 0.3%, 0.68% and 3.29% for the leaves of A. alboviolaceum; 0.58%, 0.36% and 6.89% for the red leaves, and 0.65%, 0.26% and 7.55% for the green leaves of M. expansa. The mineral content in the leaves of both plants remains dominated by the high concentration of potassium (K), calcium (Ca), Magnesium (Mg), Manganese (Mn) and Iron (Fe). Conclusion: The high content of phenolic compounds and essential trace elements makes the leaves of M. expansa and A. alboviolaceum potential candidates to alleviate several health problems in Africa in general and particularly in the Democratic Republic of Congo.

ABSTRACT

Background: Dyspnoeic in the emergency department with multiple co-morbidities is a diagnostic challenge. Approximately 15-20% of acute dyspneic in the Emergency Department due to ADHF (acute decompensated heart failure) are misdiagnosed. B-type peptide (BNP) and its amino-terminal fragment (NT-proBNP) accurately identify HF in dyspnoeic patients. In the general population with dyspnoea, plasma pro-BNP concentrations are increased in left ventricular dilatation, hypertrophy, systolic dysfunction, or diastolic dysfunction but are unaffected by pulmonary dysfunction. Aims and Objectives: To study the relation between NT pro-BNP & echocardiographic findings in acute dyspnoeic patients, and the relation between NT pro-BNP and In-hospital Mortality. Materials and Methods: Source of data- Patients admitted to the Emergency Room or cardiac intensive care unit with a history of acute dyspnea in a tertiary cardiac care center in south India, meeting inclusion & exclusion criteria, were studied. Results: The study population is predominantly constituted of the elderly population. The most common co-morbid condition was hypertension. The present study uses an NT pro BNP level of 900pg/ml as the cut-off level. 78 patients were positive for the test. Echocardiography showed that 58% had LV systolic dysfunction, 60% had diastolic dysfunction. 42 patients had EF >55%, 10 patients between 55-45%, 33 patients between 44-30% and 15 patients had EF < 30%. Mortality rate was 6% in the whole study population. However, Mortality was seen in only NT pro-BNP positive group it was not statistically significant (p=0.46). Conclusions: NT pro-BNP correlates well with the worsening of LV systolic function; as the EF decreases, NT pro-BNP increases. Increase in NT pro-BNP levels has to be interpreted in the clinical context, and it is not a substitute for echocardiography for assessing cardiac abnormalities and dysfunction.

ABSTRACT

The study assessed the phytochemical, nutritional and antioxidant properties of a functional tea made from mixture of Utazi leaf Gongronema latifolium), Nchanwu leaf (Ocimum gratissimum) and Turmeric rhizome (Curcuma longa). The three spices were individually processed by drying and mincing before combining them at different ratios that resulted to 16 samples. The samples were generated through D-optimal mixture design using statistical software (Design Expert version 12). The samples were evaluated for phytochemical, nutritional and antioxidant properties. The fresh raw materials were destalked (for the leaves), sorted, washed, withered, oxidized for 24 h, oven dried at 60oC for 30 min, cooled and milled, and packaged. Fresh turmeric rhizome were sorted, washed blanched at 90oC for 2 min, cooled, peeled, sliced, oven dried at 60oC for 30 min, cooled, milled and packaged in teabags and high density polyethylene. The alkaloids, saponin, phenol, cardiac glycoside, flavonoid, tannin, steroid and anthocyanin content ranged from 12.00-22.00, 0.42-1.15, 11.67-15.63, 0.06-0.67, 12.67-22.00, 0.56-0.89, 1.00-1.67, 46.00-54.00 mg/100g respectively. The vitamin C and E content ranged from 3.03-4.13, 0.31-0.64 mg/100g respectively. The minerals iron and zinc content of the tea samples ranged from 3.53-4.70, 0.06-0.78 ppm respectively. The sensory evaluation score of the tea infusion of the sample AB and DA were ‘liked very much’ with the rating of 8.60 and 8.20 respectively, these samples were more acceptable to the panelist. The antioxidant scavenging activity ranged from 42.00-47.00% with sample EC exhibiting the highest level of activity, and the proportion was 70g, 24.62g, 5.38g of Utazi leave, Nchanwu leave, and Turmeric rhizome.

ABSTRACT

Cotton is the most important global cash crop which controls economy of many nations. Global sustainability of cotton yield is one of the major challenges for meeting impending threats under climate change. Though India is one among the leading countries in cotton production, the supply is not enough considering the increasing demand. Scientists across the Globe are indulged in developing new lines and cultures with capacity to produce more yields. In this context, here, we have made an attempt to study the growth, physiology, and yield traits of cotton culture - TCH 1819 before its release (now released in the name of CO 17) by different chemical treatments. Observation on the leaf gas exchange traits, leaf parenchymal cells distinguished the source sink relationship of the culture. Chemical manipulation by growth retardants reduced the gibberellins content and modified the foliage structure. By characterizing the physiological potential through manipulation by growth retardant (Mepiquat chloride (0.015 %)) increased the yield by 30%. The traits identified in this study are potential indicators in breeding programme before releasing the variety.

Background and Objective: The majority of aquatic ecosystems in the Democratic Republic of Congo are highly anthropized, which could negatively influence biodiversity. The overall objective of this study is to evaluate the influence of environmental variables on fish distribution in the flooded swamp forest of the Lake Tumba micro-basin on the Mbandaka- Research Center in Ecology and Forestry of Mabali road. Study Area: This study took place in ponds, swamps, marigots and small streams in relation to Lake Tumba in the flooded forest zone of the Mbandaka-Mabali road axis in the territory of Bikoro, Equateur Province in the Democratic Republic of Congo. Methods: Fish were collected using different fishing techniques and gears. Physico-chemical parameters were sampled using a multi-parameter Combo pH probe of the brand Hanna pH/ORP/EC/DO N� HI 9828, dissolved oxygen was measured using the Brand Voltcraft DO-100 oximeter and depth was determined using a graduated board. Several ecological indices of fish communities were determined as well as Hierarchical Ascending and Principal Component Classification analyses. Results: Twenty-two fish species belonging to ten orders, thirteen families and fifteen genera were identified. The average values of the abiotic variables show that the waters of the study area are characterized by low values of physico-chemical parameters within the ranges compatible with the survival of the local fish fauna. The canonical correspondence analyses reveal that five environmental variables (temperature, depth, dissolved oxygen, sandy bottom and sandy-muddy or gravelly bottom) would explain the abundance and distribution of fourteen fish species in the Longonye site. In contrast, turbidity, conductivity, pH, muddy bottom, and bottom littered with plant debris influenced the abundance and distribution of eight fish species at the Hongo, Ilungu, Lotende and Membe sites. Scoop fishing, stump removal, and the use of ichthyotoxic plants were the main anthropogenic activities that could compromise the fish fauna in the study area. Conclusion and Outlook: Promoting sustainable local fishing methods would help preserve the fish fauna studied and limit the deterioration of fish habitats.

ABSTRACT

Mobile application-based technology was used to study medication compliance and influence on cardiovascular lifestyle in ischemic heart disease (IHD) patients who undergo percutaneous transluminal coronary angioplasty (PTCA). The Mobile-app was made available at Google play store; the app was downloaded and instructions to use were thought to patients by the research team during hospitalization in the study group. A total of two hundred patients with ischemic heart disease who underwent PTCA were enrolled into the study. The control group had telephonic follow up at periodic interval. Both groups had clinical follow ups at appropriate time intervals. Higher medication compliance, smaller number of adverse events and regular follow ups were significantly more in mobile-app group than in control group.

Aims: Sudden cardiac death (SCD) continues to be a devastating complication amongst survivors of myocardial infarction (MI). Mortality is high in the initial months after MI. The aims of the INSPIRE-ELR study were to assess the proportion of patients with significant arrhythmias early after MI and the association with mortality during 12 months of follow-up. Methods: The study included 249 patients within 14 days after MI with left ventricular ejection fraction (LVEF) _x0001_35% at discharge in 11 hospitals in India. Patients received a wearable external loop recorder (ELR) 5 ± 3 days after MI to monitor arrhythmias for 7 days. Results: Patients were predominantly male (86%) with a mean age of 56 ± 12 years. In 82%, reperfusion had been done and all received standard of care cardiovascular medications at discharge. LVEF was 32.2 ± 3.9%, measured 5.1 ± 3.0 days after MI. Of the 233 patients who completed monitoring (7.1 ± 1.5 days), 81 (35%) experienced significant arrhythmias, including Ventricular Tachycardia/Fibrillation (VT/ VF): 10 (4.3%); frequent Premature Ventricular Contractions (PVCs): 65 (28%); Atrial Fibrillation (AF): 8 (3.4%); chronic atrial flutter: 4 (1.7%); 2nd or 3rd degree Atrioventricular (AV) block: 4 (1.7%); and symptomatic bradycardia: 8 (3.4%). In total, 26 patients died. Mortality was higher in patients with clinically significant arrhythmia (at 12 months: 23.6% vs 4.8% with 19 vs 7 deaths, hazard ratio (HR) ¼ 5.5, 95% confidence interval (CI) 2.3 to 13.0, p < 0.0001). Excluding 7 deaths during ELR monitoring, HR ¼ 4.5, p < 0.001. Conclusion: ELR applied in patients with acute MI and LV dysfunction at the time of discharge identifies patients with high mortality risk.
